This project, funded by the Canary Institute of Health Research Foundation (FIISC), began ten months ago and has already assisted over two hundred patients. Through a multidisciplinary team led by physiotherapists and liaison nurses, it conducts group sessions of multicomponent exercise and health education to improve strength, balance, nutrition, and prevent falls.
Since its launch, more than one hundred group sessions have been held, and four hundred comprehensive assessments have been completed. Preliminary results show a significant sixty percent reduction in falls, improvements in mood, decreased chronic pain, and a reduction in the need for healthcare consultations.
The program, initially implemented in six basic health zones in Tenerife, has expanded to include the Elderly Residence of the Canary Foundation Casa de Acogida Madre del Redentor. The new addition in Los Silos-Buenavista adds a study population of 250 users who meet the profile of frail or pre-frail patients over seventy.
The primary objective of FRAIL +AP is to enhance the quality of life and prevent disability in frail seniors through coordinated, self-guided, and personalized interventions. The program promotes active group exercise and health education over twelve weeks, with subsequent follow-up to ensure adherence.
Furthermore, it addresses issues such as unwanted loneliness, social risk, and community participation, aiming to create protective and active health environments in the participating municipalities. The Primary Care Management considers this project a priority for maintaining the autonomy and combating the social isolation of the most fragile seniors.
