The Tenerife Primary Care Management has launched a pilot project in the Tacoronte Basic Health Zone to strengthen care and continuity of services in the island's socio-sanitary centers.

This project, which began its pilot phase last April, features a multidisciplinary unit comprising a doctor, a nurse, and an administrative professional. Its main objective is to bring healthcare closer to the patient's environment, avoiding unnecessary travel and improving care quality through a proactive approach focused on prevention and inter-level coordination.
Professionals from this unit recently participated in a training activity at the Residencia Fundación Casa Acogida Quesada Sánchez- Fundación Gerón in Tacoronte. They addressed recommendations on nutritional management, unwanted loneliness, and fall prevention for staff, residents, and day center users, as well as for individuals participating in Fundación Gerón's 'Unwanted Loneliness' program.
The unit focuses on health promotion, health education, and the prevention of geriatric syndromes such as falls, dysphagia, or malnutrition. In the Tacoronte Basic Health Zone, the unit covers six residential centers serving a total of 337 individuals, conducting scheduled visits, addressing clinical incidents, monitoring complex chronic patients, and providing training to residential staff.
This model aims to reduce avoidable referrals to hospital emergency services and improve the resolution of health problems directly within residences, benefiting both patient quality of life and healthcare system efficiency. The Tenerife Primary Care Management is thus advancing the transformation of the care model towards a more accessible and person-centered system.
The project, named UARGPTF, is initially conceived as a pilot experience to evaluate its outcomes and consider its progressive extension to other Basic Health Zones with a higher concentration of socio-sanitary centers.
