This strategy, involving eight healthcare centers across the country, aims to prevent patients from stopping their medication during the first year after a cardiac event. The program specifically targets individuals with atherosclerotic vascular disease (EVA) resulting from an acute coronary syndrome.
The care model includes intensive support lasting between twelve and fifteen months. It facilitates the transition between the hospital and primary care settings by incorporating a case management nurse and digital tools that enable more comprehensive clinical monitoring and seamless communication between healthcare professionals.
Beyond clinical follow-up, the project focuses on patient education and empowerment, encouraging individuals to take an active role in managing their own health. The initiative is endorsed by the Spanish Society for Healthcare Quality and the Spanish Society of Health Executives.
Locally, the hospital's Cardiology department has operated a Cardiac Rehabilitation Unit since 2022. This unit treats over two hundred patients annually and utilizes early discharge protocols, allowing selected patients to return home within 48 hours under a continuous telephone monitoring system.
