The Canary Islands Health Department, through the General Directorate of the Patient and Chronicity of the Canary Islands Health Service (SCS), has reported on the results of the AP-Cuida2 program. From January to June 2024, the program has registered 177,578 patients administratively discharged with a Nursing Care Report (ICE).
Of these patients, a total of 176,343 have been scheduled to see their reference Nursing or Midwifery professional at their health center, representing 99% of the total. By Health Area, the distribution of scheduled patients is as follows: 59,258 in Gran Canaria, 50,266 in Tenerife, 17,550 in Lanzarote, 9,272 in Fuerteventura, 11,207 in La Palma, 1,734 in La Gomera, and 1,030 in El Hierro.
The AP-Cuida2 program is a pioneering initiative designed to facilitate the continuity of care for patients after hospitalization. Its objective is to optimize care, manage post-discharge processes, prevent hospital readmissions and unnecessary emergency visits, and address medication-related issues, proving particularly beneficial for chronic and elderly patients.
One of the primary goals of the General Directorate of the Patient and Chronicity is to ensure continuity of care between hospital and primary care professionals, making sure that relevant clinical information and necessary care are accessible to all professionals involved in the patient's treatment.
AP-Cuida2 aims to guarantee patient safety and quality of care, while also reducing travel, improving access to health services, and facilitating the monitoring of their progress after discharge.
This program received an honorable mention at the XXXI edition of the National Informatics and Health Awards 2025, organized by the Spanish Society of Informatics and Health (SEIS), in the category for projects applying information and communication technologies (ICT) in the healthcare or social-healthcare field.
